A soft tissue injury is an injury to parts of the body that include muscles, tendons, ligaments, skin, fat and fascia, and make up 75-85% of the body’s mass.

What Are Soft Tissues?
Soft tissues make up around 75–85% of the body’s mass and include muscles, tendons, ligaments, skin, fat, blood vessels and nerves. Of course, all of these tissues are important, but this article focuses on the structures most commonly injured during everyday activities and sport:
Muscles
Muscles generate force to create movement and stabilise joints. Whether you’re climbing stairs, lifting shopping bags or playing sport, your muscles are constantly adapting to the demands placed upon them.
Tendons
Tendons connect muscles to bones. They transmit the force produced by muscles, allowing movement to occur efficiently. Healthy tendons are incredibly strong, but they can become irritated or overloaded if demands increase faster than they can adapt. Importantly, they have reduced blood supply and therefore recover, adapt, and heal slower than other tissues.
Ligaments
Ligaments connect bone to bone and provide stability around joints. They help prevent excessive movement while allowing normal joint motion. Sprains occur when ligaments are stretched beyond their capacity.
Although these tissues have different roles, they all respond to injury using similar biological healing processes.
What Is a Soft Tissue Injury?
Soft tissue injuries can happen suddenly during a specific incident (an acute injury) or develop gradually when a tissue is repeatedly exposed to excessive load beyond its capacity (often called an overuse injury), or perhaps more commonly than you may realise, injuries can occur when the tissue is not strong enough for the load being repeated (we consider this an underprepared injury – an important clinical reasoning differentiation that we will return to later).
In reality, the distinction isn’t always clear. Many people experience pain after a seemingly minor movement because the tissue has been gradually overloaded over weeks or months before symptoms finally appear.
Understanding the type of injury is an important first step in deciding the most appropriate treatment and rehabilitation plan.
Muscle Strains
A muscle strain occurs when muscle fibres are stretched or contracted (strained) beyond their capacity, resulting in microscopic or more significant tearing.
This often happens during activities that require rapid acceleration, deceleration or lifting, particularly if the tissue is fatigued or unaccustomed to the demands being placed upon it.
Common examples include:
- sprinting
- lifting a heavy object
- changing direction quickly during sport
- slipping or missing a step.
Muscle strains vary considerably in severity. Mild injuries may only involve small numbers of muscle fibres and recover within a few days or weeks (low grade), whereas more severe injuries can involve partial or complete tears (high grade) requiring several months of rehabilitation.
Ligament Sprains
A sprain occurs when a ligament is stretched or torn, often following an unexpected twist or impact.
Examples include:
- rolling an ankle
- twisting a knee while changing direction
- falling onto an outstretched hand
- awkward sporting collisions
Like muscle injuries, ligament sprains are commonly graded according to severity, helping clinicians estimate healing time and guide rehabilitation.
Tendon Injuries
Tendons are remarkably strong structures, but they adapt relatively slowly compared with muscles.
Many tendon problems develop because the tendon’s load has exceeded its capacity over time. This may occur after a sudden increase in training, repetitive occupational activities or a gradual reduction in the tendon’s ability to tolerate load. Common examples include:
- Achilles tendon pain
- tennis elbow
- patellar (kneecap) tendon pain
- rotator cuff tendon problems.
Although tendon pain can sometimes settle with rest, long-term recovery usually depends on gradually rebuilding the tendon’s capacity through appropriately prescribed exercise.
Bruises (Contusions)
Not all soft tissue injuries involve stretching or overloading.
A direct blow to a muscle or other soft tissue can cause bleeding beneath the skin, resulting in a bruise (contusion). Although these injuries often look dramatic because of swelling and discolouration, many recover well with appropriate management and gradual return to activity.
How Do Soft Tissue Injuries Heal?
One of the most reassuring things to remember after an injury is that your body already knows how to heal and that this process begins almost immediately after tissue is damaged. Our role is to help create the right conditions for that healing to occur as efficiently as possible.
Although muscles, tendons and ligaments have different structures and healing times, they all repair themselves through three overlapping phases.
Phase 1: Bleeding & Inflammation – Setting the Foundations
An injury to a soft tissue with a reasonable blood supply (such as a muscle), will usually result in some bleeding. This may involve external blood loss if the skin is also damaged or bruising beneath the skin if not. This is a normal part of injury and kick-starts the healing process. Bleeding allows the assembly of essential blood cells within the injured tissue and releases growth factors – special chemical signals – to trigger the repair response. Within seconds or minutes, the blood cells then start to clump together and clot, protecting the wound, preventing further blood loss, and providing a scaffold to keep the clot in place whilst the injury heals.
Inflammation often gets a bad reputation, but it’s also an essential part of recovery. Tiny blood vessels become more permeable (therefore increasing the volume of the tissue), allowing specialised cells and proteins to reach the damaged tissue. These cells begin removing injured tissue and releasing chemical signals that coordinate repair.
During this stage, you may notice:
- pain
- swelling
- warmth
- bruising
- stiffness
- reduced movement.
These symptoms can be uncomfortable, but they are often a sign that the body’s repair mechanisms have been activated.
The aim during this phase isn’t to eliminate every symptom. Instead, it’s to protect the injured area while maintaining as much comfortable movement as possible.
Primary Goal: Protect the tissue to create an optimal healing environment. During this phase we want to allow the body to respond to the injury so that the subsequent phases can proceed effectively. It is therefore important to respond to the symptoms and reduce the demands on your body during this initial period: think ‘less is more’.
Phase 2: Repair (Proliferation)
Once the initial clean-up process is underway, the body begins producing new tissue. Think of this as rebuilding after structural damage. New collagen fibres are laid down to reconnect injured tissues, restoring continuity and providing an initial framework for healing.
At this stage, however, the repair tissue is still relatively weak (think of a scab shortly after a skin laceration… it can come off easily and start the healing process all over again). The collagen fibres are organised quickly rather than efficiently, meaning the tissue isn’t yet ready to tolerate high levels of stress. Although symptoms often begin improving during this phase, the underlying tissue is still maturing.
Primary Goal: Start to return to low intensity movements and exercise – still protecting the tissue but with an effort needed to support the mind and body towards normal function again (as pain allows – some mild discomfort is ok, but we want to avoid increasing pain with activity here).
Phase 3: Remodelling – Building Stronger Tissue
The final stage of healing is often the longest – and critical to making a full recovery. (Using the skin laceration example, this is the development of the scar and for milder injuries, the return to normal healthy skin again – the difference with muscles, tendons and ligaments is that they need more load to recover optimally).
Over the following weeks and months, newly formed collagen gradually becomes stronger, more organised and better aligned with the direction of the forces placed upon it, exposing the healing tissue to the right amount of load at the right time. This encourages the new tissue to adapt, becoming stronger, more resilient and better prepared for the demands of everyday life or sport.
Without progressive loading, healing tissue may regain continuity but not necessarily optimal strength or function. Your physiotherapist can help you by designing a plan that speeds up your recovery and reduces risk of re-injury.
How Long Does a Soft Tissue Injury Take to Heal?
This is one of the most common questions we hear in clinic, but it’s also one of the hardest to answer.
The honest answer is: it depends.
Two people with what appears to be the same injury can recover at very different rates. That’s because healing is influenced by much more than the tissue itself.
Recovery isn’t about waiting for tissues to heal, it’s about helping your body adapt to the demands of the life you want to live.
Recovery depends on factors such as:
- the type of tissue involved (muscle, tendon or ligament)
- the severity and location of the injury
- your age and general health
- previous injuries
- your activity levels before the injury
- your rehabilitation programme
- sleep, nutrition and stress
- your goals and the physical demands of your work, sport or hobbies.
Rather than thinking about recovery as a fixed timeline, it’s often more helpful to think of it as a journey with several milestones.
Recovery Takes Time, But It Isn’t Just About Time
Every injury is different, and recovery timeframes vary. Scans provide us with method of grading an injury, and this can provide a guide for recovery times. although this is not always necessary, and we are usually able to identify whether your injury fits into a low-grade or high-grade category following a clinical assessment. It is useful to consider the following timeframes as a general guide:
Low–grade Muscle and Ligament injuries: 1 week to a month
High-grade Muscle and Ligament injuries: 1 month to 3+ months
Tendon or Bony injury: 6 weeks to 6-12 months
These timeframes describe the biology of healing. Returning to full function often takes longer because your strength, fitness, confidence and movement patterns also need to recover. This highlights the importance of working with a trusted physiotherapist that can help you safely navigate this complexity.
Why Soft Tissue Injury Healing Isn’t the Same as Full Recovery
One of the biggest misconceptions about injury rehabilitation is that once a tissue has healed, you’re ready to return to everything you were doing before.
Unfortunately, it’s rarely that simple.
Imagine taking the cast off a broken wrist. The bone may have healed, but the muscles have weakened, the joint is stiff, and everyday tasks can still feel surprisingly difficult.
The same principle applies to soft tissue injuries.
Although the injured tissue may have largely repaired itself, several other factors often need to recover too, including:
- muscle strength
- balance and coordination
- flexibility
- cardiovascular fitness
- confidence in movement
- tolerance to the specific demands of work or sport.
In other words, tissue healing is only one part of rehabilitation.
Returning to running after an Achilles tendon injury, for example, requires much more than simply waiting for the tendon to heal. The tendon needs to regain its ability to store and release energy, while the calf muscles, balance systems and running mechanics also need to be prepared for the demands of running again.
This is why rehabilitation continues even after pain has settled.
What Can You Do to Optimise Soft Tissue Injury Recovery?
Although you can’t rush biology, there is a great deal you can do to support your body’s natural healing processes.
Protect the Injury, But Keep Moving
During the first few days after an acute injury, it’s normal to experience pain, swelling and reduced movement. Your body is encouraging you to protect the injured area while the earliest stages of healing take place.
However, protection doesn’t usually mean complete rest.
For most soft tissue injuries, gentle movement within comfortable limits helps maintain joint mobility, reduce stiffness and preserve function without disrupting healing. The key is finding the right amount of activity: not too much, but not too little either.
Manage Pain Sensibly
Pain relief can be helpful if discomfort is preventing you from sleeping, moving comfortably or completing your rehabilitation exercises.
Depending on the stage of healing and your individual circumstances, this might include:
- simple pain-relieving medication (where appropriate)
- temporary use of ice or heat
- activity modification
- gentle movement
Ice is often used to reduce pain during the early stages after injury. While it may help control symptoms, current evidence suggests it should not be relied upon to accelerate tissue healing.
Heat can sometimes be useful later in recovery, particularly if surrounding muscles become tight or protective, helping improve comfort before movement or exercise.
The goal isn’t to eliminate every sensation of pain, but to make movement and rehabilitation achievable.
Gradually Reload the Tissue
As pain settles and healing progresses, the focus shifts from protection to rebuilding.
This is one of the most important stages of rehabilitation.
Muscles, tendons and ligaments become stronger when they are exposed to the right amount of load at the right time. Too little loading can delay recovery, while doing too much too soon may aggravate symptoms or increase the risk of reinjury. Rather than following a one-size-fits-all programme, rehabilitation should be progressive, and tailored to your symptoms, goals and the activities you want to return to.
How Can You Reduce Your Risk of Future Soft Tissue Injuries?
The best way to recover from an injury is, of course, not to sustain one in the first place.
While it’s impossible to eliminate injury risk entirely, there’s a great deal you can do to improve your musculoskeletal system’s ability to tolerate the demands of everyday life, work and sport, otherwise known as your body’s capacity.
Problems often arise when the demands placed upon the body exceed its current capacity.
Sometimes that’s unavoidable – such as slipping on an icy pavement or landing awkwardly during sport. More commonly, however, injuries develop because there has been a sudden change in activity. Perhaps you’ve started training for a half marathon, returned to the gym after a long break, taken on a physically demanding project at work, or simply become less active over time.
The good news is that capacity can be improved.
By gradually exposing your body to appropriately challenging exercise, your muscles become stronger, your tendons become more resilient, your bones adapt to tolerate greater loads and your cardiovascular fitness improves. These adaptations don’t just improve performance, they also make everyday activities feel easier and reduce the likelihood of overload. Should you overdo it or experience an unavoidable accident, you will be in a much better position to recover faster and stronger.
This is why rehabilitation and injury prevention are really two sides of the same coin. Both aim to improve your body’s ability to meet the demands of the life you want to live.
Build Healthy Habits, Not Just Strong Muscles
Exercise is only one piece of the recovery puzzle.
Every tissue in your body relies on adequate energy, nutrients and recovery time to repair and adapt. Research consistently shows that good general health supports better outcomes following injury, while poor sleep, chronic stress and inadequate nutrition may slow recovery and reduce exercise capacity.
Simple habits can make a meaningful difference:
Prioritise Sleep
Sleep is one of the most powerful recovery tools available.
During sleep, the body releases hormones involved in tissue repair, regulates immune function and consolidates the physical adaptations made during rehabilitation.
Most adults should aim for 7.5–9 hours of good-quality sleep each night, particularly during periods of injury recovery.
Eat to Support Healing
Your body requires energy to repair damaged tissues.
Maintaining a balanced diet that provides sufficient protein, fruit, vegetables, healthy fats and wholegrain carbohydrates supports tissue repair and provides the nutrients required for progressive rehabilitation.
Crash dieting or significantly restricting calorie intake during recovery may reduce your body’s ability to heal effectively.
Stay Hydrated
Hydration influences many normal physiological processes, including circulation, temperature regulation and exercise performance.
While hydration alone won’t heal an injury, remaining well hydrated supports your body’s normal function and makes it easier to participate in rehabilitation. Aim for 2 litres of fluids per day – or more during hot weather or when exercising.
Keep Stress in Perspective
Physical injury doesn’t occur in isolation.
Work pressures, family commitments, poor sleep and emotional stress all influence how we experience pain and how effectively we recover.
This doesn’t mean your pain is “all in your head”. Rather, it reflects the close relationship between our physical and psychological health.
Where possible, finding ways to manage stress: whether through exercise, spending time outdoors, relaxation techniques or simply asking for support, can have a positive influence on recovery.
When Should You See a Physiotherapist After Soft Tissue Injury?
Many minor soft tissue injuries improve with sensible self-management.
However, it’s worth seeking professional advice if:
- your pain is severe or worsening
- you’re unable to bear weight or use the affected limb
- swelling or bruising is significant
- the injury keeps coming back
- you’re unsure how much activity is safe
- you’re struggling to return to work, sport or exercise
- you’ve lost confidence in the injured area

Frequently Asked Questions
Should I rest completely after a soft tissue injury?
Usually not. While reducing activities that significantly aggravate your symptoms is sensible, complete rest is rarely recommended. Gentle movement and progressively increasing activity, guided by your symptoms or a physiotherapist, generally lead to better outcomes than prolonged immobilisation.
Should I use ice or heat?
Ice may help reduce pain during the first few days after an injury, while heat can sometimes relieve muscle tension and stiffness later in recovery. Neither treatment is a cure, and both should be viewed as tools to improve comfort rather than accelerate healing.
When can I return to sport?
Returning to sport depends on more than pain alone. You should have regained adequate strength, movement, confidence and the ability to tolerate the demands of your chosen activity. A physiotherapist can help assess whether you’re ready to return safely.
Do soft tissue injuries heal completely?
